Hospitals in DOWNTOWN TORONTO are now pre-registering eligible patients, aged 80 and up, for the COVID-19 vaccine, as part of the Ontario COVID-19 Vaccine Rollout. The pre-registration allows for eligible patients to submit their interest in being vaccinated – it does not provide access to book appointments at this time.

Who is this registry for?
In order to pre-register at UHN, patients must:
 

  • Be aged 80 years or older (born in 1941 or earlier)
AND
  • Be a current patient of UHN – Toronto General, Toronto Western, Princess Margaret, Toronto Rehab – Sinai Health System, Women’s College Hospital, or Toronto Grace Health Centre.
  • Please note individuals whose only visit to UHN has been for COVID-19 testing at the Toronto Western Assessment Centre, are not considered a patient of UHN.
OR
  • Live in the mid-west Toronto region (South of Eglinton to the lake, east of Dufferin St. and west of Yonge St.) Ø

Pre-registration is only available to eligible priority groups – which are determined by Ontario’s Ministry of Health (click here).
The vaccine clinic will verify information when booking vaccine appointments. Patients that do not meet the eligibility criteria will not be given an appointment.

The registry is currently only available online for patients that are regular patients of the above hospitals. Patients are encouraged to have a family member, caregiver or friend help them complete the registry.
