Preparing For Your First Visit: The Meet and Greet Visit

When you become a Umedical patient, we require you to come in for a Meet and Greet visit. On this visit we will collect medical information about your healthcare needs and you will learn about how our clinic operates. We will not be addressing specific health care needs on your first visit.

What Happens At Your Meet and Greet Visit

You will be asked to review and sign Umedical Patient Information forms in the Patient Orientation Manual, these documents include:

  • Consent for email communication, policies for medical information sharing
  • Fill in Intake Forms
  • Learn about our Uninsured Services Program
  • Learn about your responsibilities as a patient in a Family Health Organization
  • Collection of information about your previous health care providers and sign consent for transfer of records
  • Collect Pharmacy Information

How to prepare for your first Meet and Greet Visit:

  • Bring your non-expired health card
  • Be 15 minutes early
  • Fill out Umedical Patient Information forms ahead of time
  • Bring your list of medications
  • Bring list of names numbers and addresses of your previous Family Doctor or Specialists
  • Bring old medical files
  • Bring payment method for block fee program

These are services not covered by OHIP, i.e. prescription refills and forms. You can pay an annual fee to not have to worry about these charges.

  • $145/year individual
  • $215/year couple
  • $250/year family
  • $115/year senior(65+)
  • $155/year senior couple
  • (including children under 21 and residing at the same address)

Location

We are conveniently located inside the Rosedale Medical Centre, just across the entrance to the Sherbourne Subway Station.

Parking

There is paid surface parking right behind the building, access it through Selby Street, which is located one block south of Bloor.
Alternatives for parking is Public Parking on Selby Street just West of our building.
There is limited green P parking spots along Selby St, Linden St and Huntley Street

If you are being dropped off or picked up (by wheel-trans or otherwise), please have your driver drop you off at the entrance of the Rosedale Medical Centre.

Payment information

As you may know, the Ontario Health Insurance Plan (OHIP) pays for most of your medical visits to our office.
There are, however, uninsured services, which OHIP does not cover, such as prescription refills by phone or medical forms for insurance.
While you can pay for these services on an as needed basis, we also offer an Uninsured Medical Service Plan as a more convenient and cost-effective way to manage these charges. Read more details about the Uninsured Service Plan on our website, or feel free to ask any of our staff for more information about this option.
