Dear Patients,

It is with mixed feelings that I am writing to inform you that on June 6th 2025 I am leaving UMedical, to change my career focus to Palliative Care. This unfortunately means that I will no longer be able to be your primary care provider.

Making sure you have ongoing access to primary care is my utmost priority, and I have spoken with several local doctors to ensure that you have high quality care going forward. As such, following my departure your care will automatically be transferred to Dr Barbara Szopinski, whose office is a one minute walk from UMedical at 366 Bloor St. East.

Like myself, Dr Szopinski trained as a family doctor in the UK, and she is extremely passionate about providing holistic, evidence based care for her patients. Having spoken at length with her, I am extremely confident she will give excellent care to you.

What You Need To Do:

Please sign the attached form or call us to discuss how you want us to handle your charts before May 31,2025 .If you miss this deadline and still want to be transferred to Dr.Szopinski you will have to pay for the notes/records transfer .

If for any reason you DO NOT wish to have your file transferred, and would rather find a new family doctor yourself, please contact UMedical as soon as possible.>Dr Szopinski will be reaching out to you in the near future with an email to introduce herself. Until June, I will continue to be working the same hours and am happy to see you for any healthcare needs you may have.
