b) Rosedale Radiology, for x-ray or ultrasound ☎️ call 416 960-0177
📍Requisitions for x-ray / ulstrasound/ Mammogram/ Bone Density Scan
It is YOUR RESPONSIBILITY to book your appointment for an x-ray or ultrasound
👉 Your doctor will inform you if she will:
📧 Email you the requisition, if this is the case, please print it and take to your appointment
📑 Fax it directly to the radiology location, if this is the case, you do not need to do anything
📨 Mail via Canada post, if this is the case please take it to your appointment.
